AFFIDAVIT OF HEALTH

I declare the following: 

I  have been clearly reported the health medical questionaire to get the health certificate in order to provide my Spanish Visa to the best of my medical knowledge, especially the question 14th referred to the infectious diseases dangerous for the Public Health according to the IHR (International Health Regulation) of 2005.

I also know this medical certificate is officially approved by the Ministry of Health and the official Medical college Organization throughout the Spanish national territory. This certificate is based on an affidavit of health formulated in a complete medical questionnaire prepared for this purpose. I have understood that in the case I, as applicant, would respond untruthfully or omitting data of the real health status, I may incur a crime of documentary falsification before the Spanish authorities.

For this reason, the Visa and the Permit for residence granted may be withdrawn. I would have leave the country within the period established in their case by the Immigration Law or submit a voluntarily extensive medical examination and at their expenses, ordered and supervised by the same doctor who signs the current certificate, which will determine if there was an inaccuracy in the certificate due to error or falsehood.

Legally,I have been reported that the visa and residence permit is treated as an extension of the application of the proportional rule of article 10 of the LCS (Law 50/1980, of October 8, on Insurance Contracts) in this case, for obtaining the Visa and consequent Residence Permit in with a false affidavit could result in the termination the contract due to inaccuracy as one of a cause for termination of article 20.8 of the LCS.
